Aetna Golden Medicare Plan ( Medicare + Choice HMO plan): only available in certain areas of California, New Jersey, New York and Pennsylvania.

You will only receive coverage in the network of doctors, specialists, and hospitals. You will need to select a network primary care physician (PCP), an internist or general practitioner who knows your medical history and coordinates all of your covered care.

Referrals NOT needed for:

• Routine visits to your PCP
• Ob/Gyn care
• Annual Mammograms
• Immunizations
• Emergency and Urgent Care

Eligibility- Entitled to Medicare Part A and enrolled in Medicare Part B, live in the service area

Aetna Golden Choice Plan (Medicare + Choice plan w/ an out-of-network benefit): only available in certain areas of New Jersey, Maryland, Pennsylvania.

Accessing care- NO REFERRALS REQUIRED

• Visit any licensed out-of-network physician, and pay applicable deductible and coinsurance
• Visit any in-network physician and pay applicable specialist copay
• Visit your primary care physician (PCP) and pay the applicable PCP copay
