Small Group (2-50 employees) Health Insurance
The following is a list of the basic types of health insurance that are relevant to small business owners:

1. Managed Care

Most people today who obtain health insurance through their employers are in some type of managed care plan. These are some of the basic characteristics shared by managed care plans:

  • Contracts with selected doctors, hospitals, and other providers to deliver a set of comprehensive health care services to its members
  • Programs for quality assurance and utilization review
  • Lower costs for people who use providers that are part of the network

A) Health Maintenance Organizations (Click on HMOs for more info)

HMOs provide a set of basic and supplemental health care services to its members for a fixed premium. Usually, there are no deductibles and no, or minimal, copayments. There are several types of HMOs, including the following:

  • Staff model, where providers are directly employed by the HMO
  • Group model, where medical groups contract with the HMO
  • Independent practice association (IPA), where the HMO contracts with independent physicians, either individually or through associations. IPA physicians often have contracts with more than one HMO
  • Network model, where the HMO has arrangements with many providers in a given area
B) Preferred Provider Organizations (Click on PPOs for more info)

A PPO is usually made up of groups of hospitals and providers that contract with employers, insurers, or others to provide health care services to members for a negotiated discounted fee. Some of the different types include:

  • Third-party-payer PPOs, such as commercial insurers and Blue Cross and Blue Shield plans
  • Hospital-sponsored PPOs, which consist of a network of institutions and many of their physicians
  • Physician-sponsored PPOs, developed by local medical associations or groups of physicians
  • Employer or labor-sponsored PPOs, which contract directly with providers on behalf of employees or members
  • PPOs developed by nonphysician providers, such as dentists, optometrists, pharmacists, chiropractors, and podiatrists through professional associations
